on the provision of financial assistance during the period of the suspension of classes at a university resulting from the SARS-CoV-2 virus threat
Following the decision to suspend didactic classes at the Lodz University of Technology and in accordance with the recommendations of the Minister of Health and the Chief Sanitary Inspector recommending to limit direct contacts between persons to the necessary minimum, as of 12 March 2020 I am introducing the following changes in the procedure for applying for financial aid by students and PhD students until further notice:
1. students/doctoral students applying for a social scholarship, assistance and scholarship for the disabled shall submit the relevant applications only through:
- sending legible scans or photos of the original printed filled in and signed application form and scans or photos of all required documents (certificates, statements, etc.) as attachments to an e-mail message sent from an individual account in the TUL e-mail system and sent to res@adm.p.lodz.pl
and then
- immediately sending via the Poczta Polska [Polish Post Office] a letter containing the originals of the above-mentioned documents to the address: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116 90-924 Łódź.
2. Students/doctoral students applying for the Rector's scholarship shall submit relevant applications in the period from 14 to 20.03.2020 exclusively through:
- sending legible scans or photographs of the original printed completed and signed application form and scans of all required documents (certificates, declarations, etc.) as attachments to an e-mail message sent from an individual account in the TUL e-mail system and sent to res@adm.p.lodz.pl.
and then
- immediately sending via the Poczta Polska [Polish Post Office] a letter containing the originals of the above-mentioned documents to the address: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116 90-924 Łódź.
3. Students/doctoral students who want to settle health insurance matters submit the relevant forms only through:
- sending legible scans or photographs of the original printed, completed and signed ZUS print and a statement, as attachments to an e-mail message sent from an individual account in the TUL electronic mail system and directed to res@adm.p.lodz.pl
or
- sending via the Poczta Polska [Polish Post Office] a letter containing the originals of the above-mentioned documents to the address: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116 90-924 Łódź.
In individual situations, the student/doctoral student may be asked by e-mail or telephone to appear in person at the TUL Benefits Service Section, ul. Żeromskiego 116 in Łódź, building A-33, entrance E, fourth floor in order to complete or clarify the details of the documentation.
