Ladies and gentlemen, Further to the announcement of January 11, 2021, the following details are provided to assist current and former employees, doctoral students, and students in facilitating participation in the COVID-19 vaccination process.
1. The schedule presented by the Government of the Republic of Poland is as follows:
a. people over 80 - registration from January 15, 2021;
b. people who are 70 years of age - registration from January 22, 2021;
c. beginning of vaccination process for both of the above groups on January 25, 2021, with priority given to the 80+ group;
d. vaccination dates are determined on a first-come, first-served basis;
e. the remaining dates have not yet been determined by the Polish Government.
2. Each person mentioned in point 1 can register for the vaccination by:
a.registration in the Patient's Internet Account;
b. completing the form at any vaccination point;
c. filling in the form on the Wikamp platform at: https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890, and then submitting a written declaration on the day of vaccination.
d. filling in the form from the attachment and deliver it to the Centre for Diagnostics and Laser Therapy for vaccination at the vaccination point at Lodz University of Technology in the Centre for Diagnostics and Laser Therapy.
3. Vaccinations carried out by the Centre for Laser Diagnostics and Therapy of the Lodz University of Technology will take place according to the order of applications, after prior determination of the exact vaccination date in order to reduce queues in the CDiTL TUL building.
4. People under 70 years of age, interested in vaccinations, may use the solutions presented in points 2.c. And d., Provided that the date of vaccination will be set only after vaccination is started for groups other than those for people over 70 years of age.
5. All information updates will be sent to you after publication by the Government of the Republic of Poland.
6. In the case of former employees and retirees of Lodz University of Technology, please apply point 2.d.
