Ladies and gentlemen,
I would like to inform you that the registration for vaccinations against COVID-19 for employees of Lodz University of Technology is still ongoing. I kindly remind you that:
1. After completing the form available at: https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890, you will be registered for the first possible vaccination date at the Laser Diagnostics and Therapy Centre of Lodz University of Technology. You will receive information about the date of vaccination via SMS.
2. People who applied via the form on the Wikamp portal are entered into the Online Patient Account system in accordance with the government's registration schedule at the time of the appointment of the vaccination date.
3. If any of you have completed the above form so far, you do not have to fill it in again.
4. If any of you born before 1962 signed up for vaccination by filling out the above form, and has not yet received information about the date of vaccination, please contact us by e-mail at office.rector@adm.p.lodz.pl.
5. Vaccinations take place in accordance with the schedule specified by the Government of the Republic of Poland. The April registration schedule is available at: https://www.gov.pl/web/szczepimysie/wczessze-terminy-szczepien-osoby-60… single-).
6. The Ministry of Health responded to one of the appeals regarding the inclusion of non-teachers working at universities in the vaccination program as a priority. I encourage you to read the attachments to the announcement.
7. It is possible to vaccinate people not employed at Lodz University of Technology at the Laser Diagnostics and Therapy Centre of Lodz University of Technology. In this case, please contact CDiTL directly.

April 13 - born 1963,
April 14 - born 1964,
April 15 - born 1965,
April 16 - born 1966,
April 17 - born 1967,
April 19 - born 1968,
April 20 - born 1969,
April 21 - 1970,
April 22 - born 1971,
April 23 - born 1972,
April 24 - born in 1973.
The above-mentioned age groups will receive information about the date of vaccination on individual days mentioned above.
The dates of registration of subsequent years will be determined in accordance with the schedule agreed with the Ministry of Health.
